The APA service will assist employees with customizing a benefits package designed to ensure employees and/or their dependents/beneficiaries have knowledge of the services that are needed in every area of their Core 4 Benefits™ package. The benefits that have been included are listed as follows: Retirement, Healthcare, Disability, and Life Insurance. I understand that this service is for the benefit of the employee(s) and their dependents/beneficiaries. I authorize B4A & APA Representatives to work with my Human Resources Department, Approved Vendors and any additional Companies required to customize my basic benefits package.
